SNAP-ED

What is Snap-ED?

What is
Snap-ED?

The Supplemental Nutrition Assistance Program Education (SNAP-ED) is a federally funded grant program that helps participants eat healthier and stay active.

We hope to give Georgians the tools where they live, work, play, learn, shop, and eat so that they can make the healthy choice, the preferred choice.

What is the SNAP-Ed Focus?

SNAP-ED works to teach people about healthy eating and meaningful movement. For those with nutrition-related diseases, such as obesity or diabetes, the program hopes to delay or even prevent them through creating long-lasting healthy habits.

Who Qualifies for Classes and Services?

  • People currently receiving SNAP, WIC, TANF or any other means-tested federal assistance program
  • People at sites such as food pantries, food banks, soup kitchens, public housing and SNAP/TANF job readiness programs
  • People that live in a low-income census tract or zip code
  • People that are eligible for public assistance programs and are currently not receiving public assistance

Community Health Assessment / Community Health Improvement Plans

Community Health Assessments (CHA) and Community Health Improvement Plans (CHIP) have been created for the three counties we serve.

The community-wide health assessment, which examines the county’s health status, is intended to help shape the collaborative efforts to improve the health of residents.

The community health improvement plan describes how the Health Department and a variety of community partners will work together to improve the health of the population through various projects and programs.

Access each county’s CHA and CHIP below:
