PREVENTION

Nearly one out of three Americans has prediabetes, most do not know it. When you have prediabetes your blood glucose (sugar) level is higher than normal but not high enough to be diagnosed as diabetes. High blood sugar raises your risk of type 2 diabetes, heart disease, and stroke. Without weight loss or moderate physical activity, many people with prediabetes can develop type 2 diabetes within 5 years.

  • Your chances of having prediabetes go up if you:
  • Are 45 or older
  • Are Black, Hispanic/Latino, American Indian, Asian American, or Pacific Islander
  • Have a parent, brother or sister with diabetes
  • Are overweight
  • Are physically inactive
  • Have high blood pressure or take medicine for high blood pressure
  • Have low HDL cholesterol and/or high triglycerides
  • Had diabetes during pregnancy
  • Have been diagnosed with Polycystic Ovary Syndrome

Exercise and diet play an important role in helping keep your blood sugar down and can help prevent serious complications such as heart attack, stroke, blindness, kidney failure, or loss of toes, feet or legs.

Community Health Assessment / Community Health Improvement Plans

Community Health Assessments (CHA) and Community Health Improvement Plans (CHIP) have been created for the three counties we serve.

The community-wide health assessment, which examines the county’s health status, is intended to help shape the collaborative efforts to improve the health of residents.

The community health improvement plan describes how the Health Department and a variety of community partners will work together to improve the health of the population through various projects and programs.

What’s an epidemiologist?​

Epidemiologists are public health workers who investigate patterns and causes of disease and injury. They seek to reduce the risk and occurrence of negative health outcomes through research, community education and health policy.
