The Gwinnett, Newton and Rockdale County Health Departments can provide the immunizations, screenings or record transfers your child needs to attend child care and school in Georgia.

Georgia law requires children attending a school or childcare facility be protected from certain vaccine-preventable diseases. There are additional requirements for 7th and 11th graders.

Georgia Immunization Certificate (Form 3231)

Parents will need to provide a Georgia Immunization Certificate (Form 3231) to their child’s school or child care to show their child has been vaccinated against certain preventable diseases.

If your child does not meet Georgia requirements, the Health Department can vaccinate your child with any needed immunizations.

Vaccinations start at $21
If your child receives required vaccinations at one of our health centers, you will be provided a Georgia Immunization Certificate (Form 3231) free of charge on the day of vaccination

Out of state immunization
records transfer $10

With proper documentation, out of state immunizations records can be transferred onto Georgia Immunization Certificate Form 3231.

Certificate of Vision, Hearing, Dental, and Nutrition Screening (Form 3300)

Georgians must file a completed Form 3300 with the school when a child is enrolled.

If your child needs screenings, we can perform them at the health department:

  • Hearing $10
  • Vision $10
  • Dental $15
  • Nutrition $15
  • Hearing, vision, dental and nutrition $50


Out of state immunization records transfer $10

With proper documentation, records from previous hearing, vision, and dental screening conducted within the last 12 months can be transferred to Certificate of Vision, Hearing, Dental, and Nutrition Screening (Form 3300).

Community Health Assessment / Community Health Improvement Plans

Community Health Assessments (CHA) and Community Health Improvement Plans (CHIP) have been created for the three counties we serve.

The community-wide health assessment, which examines the county’s health status, is intended to help shape the collaborative efforts to improve the health of residents.

The community health improvement plan describes how the Health Department and a variety of community partners will work together to improve the health of the population through various projects and programs.

What’s an epidemiologist?​

Epidemiologists are public health workers who investigate patterns and causes of disease and injury. They seek to reduce the risk and occurrence of negative health outcomes through research, community education and health policy.
